ABSTRACT

OBJECTIVE:To explore the weaknesses of emergency management in PIVAS of our hospital, improve the ability to handle emergencies, and provide reference for risk management in PIVAS. METHODS:Frequency and severity of emergency events were evaluated comprehensively by the workers in PIVAS. Emergency events vulnerability was analyzed in PIVAS of our hospital by calculating risk score, classify risk degree and risk control method was developed for management. RESULTS:There were 8 serious risk emergency events,5 high-risk ones,5 moderate-risk ones,4 low-risk ones and 2 mild-risk ones in PIVAS of our hospital. The high-risk events included information system and drug dispensing machine failure, cytotoxic drugs overflow, drug delivery to wrong department, etc;risk scores of these emergency events were 129.81,109.92,102.01,86.49,respectively. Through emergency event training, standard operation, regular check and other management measures, drug delivery to wrong de-partment,cytotoxic drugs overflow, failures of equipment and information systems were decreased by 81.82%(4 times vs. 22 times),75%(1 time vs. 4 times)and 63.5%(15 times vs. 40 times),which compared between first half year of 2015 and 2014. CONCLUSIONS:Based on the results of vulnerability analysis, relevant emergency plans and training exercises in departments have been improved and the occurrence of relevant emergency events can be disposed effectively.

RESULTS:288 patients were treated with antibiotics. 225 cases of prevention medication accounted for 78%;63 cases of drug for treatment accounted for 22% . Among 225 cases of prevention medication,116 cases of rational choice of drug accounted for 52%,109 cases of irrational for 48%;68 cases of suitable administration time accounted for 30%,157 cases of inappropriate for 70%;the average time of preventive medication in type Ⅰ,type Ⅱ and type Ⅲ incision surgery were(4.61? 3.69)d,(7.15?3.93)d,(6.89?2.23)d respectively;as for active treatment,166 cases of single drug consisted of 74%,50 cases of two-drug 22% and 9 cases of three-drug 4%. Unreasonable administration of drug manifested as inappropriate choice of drug, inappropriate administration time, long course of treatment and the high probability of drug combination, etc. CONCLUSION:The application of perioperative antibiotics should be managed and monitored further more.
